Wellness4Workers – Wellness Access Agreement

1. Purpose
This agreement establishes a partnership between Wellness4Workers Ltd ("W4W") and theundersigned facility ("Partner"). The Partner agrees to provide wellness access to referredparticipants under W4W’s charitable assistance model.

2. Terms of Participation
Partner agrees to accept eligible W4W referrals for participation in the program. W4W will pay thePartner directly for each approved participant under the program. No contractual service is formedbetween the donor and the Partner; participation is provided under W4W’s charitable model andfunded from W4W’s general charitable pool.

3. Insurance & Liability
Partner confirms it maintains current public liability and personal injury insurance for all users.Referred participants must complete the Partner’s standard waiver or registration process beforeaccessing services.

4. Payment & Reporting
W4W will remit payment for approved participants on a regular basis, as agreed by both parties.Partner agrees to confirm attendance and report any issues that may affect service delivery.

5. Term & Termination
This agreement is valid for the duration of the W4W pilot program unless otherwise agreed. Eitherparty may terminate this agreement with written notice.

6. General
This agreement does not create an employment, agency, or joint venture relationship. Theagreement may be updated or amended in writing by mutual consent.
